So you’re a caregiver. What now?
It all starts with organization
The single most important thing you can do to function efficiently and effectively as a caregiver is to create and maintain a comprehensive file on the person you care for. Whether it's electronic or on paper, in a box or a binder doesn't matter. What matters is that the system works for you.
Here's what you'll want to include (as relevant).
- Medical history, including any surgeries or procedures
- A current diagnosis
- Physician contact information
- Medication list (daily dosage, timing)
- Pharmacy information
- Allergies, especially to any medications
- Calendar of doctors’ appointments
- Insurance information (private, prescription plan, Medicare, Medicaid, long-term care, dental and vision)
- Living will (may include advance directives)
- Healthcare proxy (also known as durable power of attorney for health)
- Financial power of attorney
